The following requirements must be completed prior to testing and proof must be brought on the day of testing:
- Commercial License Permit (CLP) dated at least 14 days prior to testing (CLP is valid for 1 year)
- Proof of insurance for vehicle used during testing. Kirkwood does have vehicles for use but will cost an additional $150 fee
- Testers who have received their Commercial License Permit (CLP) on or after Feb. 7, 2022 must complete an Entry Level Driver Training (ELDT); if permit was received prior to Feb. 7, 2022 additional training is not required. Once complete, record of completion must be uploaded to the FMCSA Training Provider Registry (https://tpr.fmcsa.dot.gov/)
- DOT Long Form Physical within the last two years
- Pre-employment Drug Screen Screening must be completed within 10 days of the testing
- A valid medical card
- Register as a driver on the FMCSA Drug and Alcohol Clearing house (https://clearinghouse.fmcsa.dot.gov/register)
